Box Lunch Catering Orders by FAX - Instructions
 

   *   Please refer to our Catering Lunch Menu or our Salad Menu for more details on selections.

   *   Filll out our catering Fax Form in its entirety, using HEAVY BLACK INK (or, get pdf version).

   *   Be sure to indicate Delivery or Pickup. Yes, WE DELIVER! Call for details.

   *   Be sure to provide credit card name, account number and expiration date.

   *   Fax the Fax Form to City Grocers at 314-621-6855.

   *   Please call us at 314-621-0010 to verify our receipt of the fax, or if you have a question.

   *   Please allow 24 hours notice. Please call first for a same-day order special exception.
 

BOX LUNCHES - One price ($9.00 plus sales tax*) for your choice of Sandwich, Wrap or Salad, all of which include:

   *   A piece of whole fresh fruit and

   *   A large fresh-baked cookie

   *   All box lunches include utensils, napkins, salt & pepper
 

With Sandwich, also choose a side:

   *   Creamy cole slaw / rotini pasta salad / red potato salad /  
        Chips (Lays Regular, Baked Lays or Nacho Cheese)
   *   Sandwiches include lettuce and tomato (mayo & mustard on the side)

   *   Chicken Caesar Salad w/Caesar Dressing

   *   Almond Dijon Chicken Salad w/Honey Mustard Dressing

   *   Thai Turkey Salad w/Toasted Sesame Dressing
 

Add any Coke or Pepsi product (12 oz. can) or bottled water for $1 each
 
* The total sales tax rate effective July 1, 2008 is 9.741%. That equates to $.88 on a $9.00 purchase.
